Creating a Fillout Account

1️⃣ Visit Fillout.com.2️⃣ Click Get Started and choose your signup method:

Sign up with Google or Microsoft.
Or enter an email address and password.3️⃣ Confirm your account (if prompted).
4️⃣ You’ll land on your dashboard, where you can begin building forms.

Designing Your First Form

1️⃣ From your dashboard, click Create Form.

Choose from blank, from template, or connect to 3rd party programs

2️⃣ Use the drag-and-drop builder to add fields such as:

Applicant Name
Email
Phone Number
Coverage Type
3️⃣ Add your agency’s logo and colors to match your brand.
4️⃣ Save your form.

Next Step: Connecting to Momentum AMS

Once your form is ready, you’ll connect it to Momentum AMS using the Form Webhook Integration.

Important:

By default (with Advanced View OFF), Fillout will send all form fields automatically to your webhook endpoint.
If you turn Advanced View ON, you’ll have to manually add each field you want to send. Only use Advanced View if you want to customize the payload.
