How to Create Specific Service Request Types
  Service Requests in Momentum AMS allow you to submit and track changes for policies in a structured workflow. Each Service Request type captures specific details so the change can be processed accurately by the carrier or internally.

  This article walks you through how to create each Service Request type, where to find them, and what information is required for each. 


Step-by-Step: Creating a Service Request

1️⃣ Access the Service Request Form

  • From the insured’s Activity History tab, click Service Requests.

  • Click Add New at the top right.

  • In the Service Request Type dropdown, select the change you need to make.


Service Request Types

Driver – Add, Replace, Remove

Use when you need to add, replace, or remove a driver from a policy.
Required Information:

  • Driver’s full legal name

  • Date of birth

  • License number and issuing state

  • Effective date of the change

  • Action type: Add, Replace, or Remove

  • Linked policy


Vehicle – Add, Replace, Remove

Use for changes to the vehicles on a policy.
Required Information:

  • Year, Make, Model

  • VIN

  • Garaging address (must match or be linked to an insured location)

  • Effective date of the change

  • Action type: Add, Replace, or Remove

  • Linked policy


Address Change

Use when updating the insured’s primary or mailing address.
Required Information:

  • New address, city, state, and ZIP

  • Effective date of change

  • Whether this is a change for the entire insured profile or just a policy location

  • Linked policy (if applicable)


Vehicle Transfer

Use when moving a vehicle from one policy to another within the same insured.
Required Information:

  • Vehicle details (Year, Make, Model, VIN)

  • Policy transferring from

  • Policy transferring to

  • Effective date of the transfer


Certificate of Insurance (COI)

Use for issuing a certificate that requires tracking as part of a service workflow.
Required Information:

  • Certificate holder’s information

  • Linked policy(ies)

  • Description of operations / special wording (if applicable)

  • Delivery method (email, fax, etc.)


Policy Change – Other

Use for policy changes not covered by other categories.
Examples:

  • Adding an endorsement

  • Changing limits or deductibles

  • Removing coverage parts
    Required Information:

  • Detailed description of the requested change

  • Linked policy

  • Effective date


Generic

Use for requests that are not policy-specific or do not fit other categories.
Examples:

  • Requesting claim status from a carrier

  • Requesting general account information
    Required Information:

  • Clear description of the request

  • Linked insured or policy (if applicable)


Best Practice

When creating a Service Request, always link it to the correct policy when possible. This ensures the request is visible in that policy’s activity history and avoids confusion during processing.
