Test Case: Creating a Simple Zap with Google Forms and Gmail
  This test case walks through how to create a simple Zap in Zapier using Google Forms as the trigger and Gmail as the action. The workflow demonstrates how Zapier collects form data and automatically sends an email with that information. While this example uses Google apps, the same process applies when connecting Zapier to Momentum AMS. 


Creating the Google Form

  1. Open Google Drive and click New → Google Form.

  2. Name the form and add the following fields:

    • Customer Name (Short answer)

    • Email (Short answer, set the question to collect a valid email address)

    • Phone Number (Short answer, optional validation for numbers)

  3. Submit at least one test response so Zapier has data to pull when setting up the trigger.


Building the Zap

  1. Log into Zapier and click Create Zap.

  2. In the “Trigger” section, search for Google Forms and select it.

  3. Choose the event New Form Response.

  4. Click Continue.

  5. Under Choose Account, connect your Google account if it hasn’t been added already. Grant Zapier permission to access Google Forms.

  6. From the dropdown, select the form you created in Google Drive.

  7. Click Test Trigger. Zapier will pull in the sample response you submitted earlier and display the fields (Name, Email, Phone Number).


Configuring the Action

  1. In the “Action” section, search for and select Gmail.

  2. Choose the event Send Email.

  3. Connect your Gmail account to Zapier and authorize access if you haven’t done so previously.

  4. Configure the fields:

    • To – Enter your own email address for this test (or map the “Email” field from the Google Form if you want the message to send to the respondent).

    • Subject – Enter a subject line, e.g., New Form Submission from Google Forms.

    • Body – Add both text and dynamic content by clicking into the field and selecting fields from the Google Form trigger. For example:

A new form has been submitted: Name: {{Customer Name}} Email: {{Email}} Phone: {{Phone Number}}

Zapier will replace the placeholders with real data from each form submission.


Testing the Zap

  1. Click Continue and then Test & Continue.

  2. Zapier will send a real test email using the form response data.

  3. Open your Gmail inbox and verify the test email looks correct.

If the email doesn’t appear, double-check the mapped fields, the connected Gmail account, and whether your test form submission had data.


Activating the Zap

Once the test succeeds:

  • Give your Zap a clear name (e.g., Google Form → Gmail Email Notification).

  • Use the toggle in the upper right to turn the Zap On.

  • From now on, every time someone submits the Google Form, Zapier will automatically send an email with the captured details.


Extending to Momentum AMS

This workflow can be adapted to Momentum AMS:

  • Instead of Gmail as the action, select Momentum AMS and choose Create Prospect (or another action your agency uses).

  • Map the Google Form fields (Name, Email, Phone) to the corresponding fields in the Momentum AMS prospect record.

  • This allows every form submission to automatically create a new prospect in your AMS without double entry.

Final Thoughts

This test case shows the building blocks of Zapier: a trigger (Google Form submission) and an action (sending an email). Once you understand this flow, you can replace Gmail with Momentum AMS to create prospects, assign tasks, or log activities automatically.
